Let us remember that Dambinima Sambuev headed the Ministry of Health in December last year. He took part in an open competition for a ministerial position. The selection consisted of several stages: the first - acceptance and consideration of applications, the second - interview and defense of programs and, finally, testing. Its results were assessed by specialists from the Russian Presidential Academy of National Economy and Public Administration (RANEPA). The new head of the department beat out 18 other applicants. Before that, he was the director of the territorial compulsory health insurance fund.

Sambuev's predecessor, Valery Kozhevnikov, while still a minister, earned 2.9 million rubles a year earlier. At the end of September, he left the position he held for no less than 15 years. He was appointed deputy head of the plenipotentiary representative of Buryatia to the President of Russia. And in November it became known that Kozhevnikov resigned from his position in Moscow, returned to his homeland and officially began working at the Republican Oncology Center.

This fall, after 15 years of work as Minister of Health, Valery Kozhevnikov left his post of his own free will. The ex-minister, and now the chief physician of the republican oncology clinic, spoke to a correspondent of the Vostok-Teleinform news agency in an exclusive interview about the reason for this, about his new place of work and about the situation in the field of oncology.

Valery Veniaminovich, why, after 15 years of working as the Minister of Health of the Republic, did you decide to leave it of your own free will? After all, unlike other ministers, there were no special complaints about your work.

In any work there comes a time when you need to sum up some results. Working in this position, and I have already said more than once that it is not sugar, but really a firing squad, I realized that it was time for a new person with a fresh look to come to this post. You always need to move forward. After 15 years of working as a minister, even though it is an interesting and very responsible position, you get tired. I can say that I was lucky, since the government of the republic and the Russian Federation and the first president of Buryatia, and then its two heads, supported me in all endeavors in the healthcare industry.

As for the negative aspects, of course, there were those too. First of all, they were related to the material and technical base, medical equipment. Of course, there were also problems with human resources. I started working as a minister in 2002, but it was only in 2013 that the whole system became a managed structure. It was very difficult to achieve this, because each region of the republic has different opportunities, personnel potential, plus a different territory, has its own characteristics. Nevertheless, I think we managed to create the backbone of healthcare, which is working today. Thanks to this, we were able to fulfill the tasks set by the government of Buryatia, Russia, and the Ministry of Health of the Russian Federation. I can honestly say that there is a result. The only indicator that has not yet been achieved is to reduce morbidity and mortality from malignant neoplasms. Even if it does not depend globally on our actions, it remains quite high in Buryatia, although lower than in the country as a whole. So, in the Russian Federation it is 214 cases per 100 thousand population, here in Buryatia it is 182. At the same time, I would like to note that malignant and benign neoplasms are now based on the same statistics, hence these numbers.

- And yet, after 15 years, you have probably become accustomed to this way of life. Now, having changed it, do you regret it?

I went to medical school to help people. And, despite the fact that I worked for many years as a healthcare organizer, I have always been attracted and attracted to working in a practical level. I could never imagine myself without practice. For me, the relationship between administration and my professional activities has always been a priority, even if it was self-training or certification cycles, I have always believed that a good doctor is, first of all, one who systematically studies, reads, and improves. And now there are more and more opportunities for this. You can even watch seminars, conferences, and so on via the Internet, live broadcast. And I am deeply convinced that today it has become interesting to work in the practical sector. Having come here, I will bet on human resources. And here, in oncology, highly qualified specialists work.

Today the task is not only to improve work with patients, but also the atmosphere within the team. It is also necessary to improve the quality of stay of patients and medical staff themselves in a health care facility. Because jobs still leave much to be desired, and this can only be done when the material and technical base is built or improved. Therefore, I won’t be bored, there is a lot of work.

- And when you headed the ministry, did you also see patients as a cardiologist?

I accepted people and helped people. Previously, while working at an emergency medical station, I was involved in cardiology, anesthesiology and resuscitation. There were cases when I provided medical assistance, as the Minister of Health, on the roads of the republic when there were accidents. There was a case under the new head of Buryatia, when during a working trip we were in the Bauntovsky district, it so happened that we had to apply our knowledge in practice.

- Can you tell me more about this?

We made a working trip with the government of the republic to the Bauntovsky district. There, in the village of Romanovka, Alexey Tsydenov was supposed to visit a new paramedic and obstetric station and I arrived at the place 15 minutes early. And there I saw that paramedics were providing medical care to a patient; the man had the most acute phase of myocardial infarction. I joined the brigade and just at that time the head came in. I said that I couldn’t leave the patient, but it was necessary to provide assistance in transporting him to the Trans-Baikal Territory, because, as it turned out, it was from there that he came to stay with us in the north. We contacted the Minister of the Territory, disaster medicine and did everything possible to save a person’s life, and this is not the only case. Once we were going on vacation and saw an accident in the Barguzinsky district, there was an accident with young girls. Then I, too, could not drive past, we promptly called a team of doctors from the Pribaikalsky region and provided medical assistance to the victims. Well, then they, these girls, once gave me a cake for the New Year. And it’s not that I’m proud of it, it’s different, it’s my job. And as for consultations, I have always conducted them and will continue to do so, because I really love practical work and cannot live without it.

- Are there, say, three things that you are proud of that you managed to do as minister?

Why only three? (Laughs) Well, firstly, now Buryatia is known and I’m glad that in such a period of time, the Minister of Health of the Russian Federation visited us, it’s great. Secondly, we held the most important symposium, and thirdly, I am grateful that the Russian Ministry of Health has always supported the Ministry of Health of Buryatia in all its endeavors.

- Are there any actions that you are ashamed of or perhaps that you would like to correct?

It’s probably a shame that when I came to work as Minister of Health, I wanted to do everything quickly, but resolving issues requires a certain period of time. And sometimes I made mistakes in my assignments.

- You have already explained why you refused a prestigious position in Moscow, in the embassy, ​​and decided to stay in Buryatia. Tell us why you chose the cancer clinic for further work? Moreover, the clinic was run by your teacher, Alexander Petrovich Perinov?

Yes, we have known Alexander Petrovich for a very long time. When I completed my residency in 1985, he worked in an emergency hospital, that is, really, my teacher. He is a professional in his field and, despite the fact that he excellently managed the dispensary for more than 30 years, continuity was respected between us. There were other proposals, but the task was set to continue the construction of the cancer clinic and find opportunities for this. Because previously the dispensary was not included in any program; we built it with 2 billion 200 million extra-program money. Considering the importance of the oncology service, I decided to come here. Moreover, this year has been declared the year of the fight against malignant tumors.

- What are your first impressions of the new place?

Great team here. I think that now we just need to unite the team with our energy and, perhaps, deeds. I have always believed and will continue to believe that when a new person comes to the team, no matter who he is, and especially a former minister, the team always looks at him with wariness. And in order to become one of your own, you just need to show through your actions how good a leader you are, how much you can stand up for your colleagues and do your best. It is necessary to resolve issues not only with the presence of health workers at their workplace, but also probably some everyday issues, household problems. When, for example, we have specialists who cannot place their children in kindergarten, we definitely need to help, because, as a rule, these are young specialists whom we don’t want to lose at all. And I’m not just talking about doctors now. For example, I have always believed and will continue to believe that the head nurse, head sister and hostess sister are the people who make our patients’ stay comfortable, and their condition directly depends on how they work.

You say that the situation with cancer is difficult in Buryatia. How can a person understand that he has cancer? How to identify it in the early stages? Surely in this case the chances of recovery are much greater?

Firstly, I believe that a person is his own boss and should monitor his own health, because doctors are doctors, and life is his. Meanwhile, despite all the medical examinations that exist, despite the interaction of employers with outpatient clinic services, who are themselves interested in undergoing medical examinations, in most cases people with already advanced diseases come to us. Because they usually go to the doctor when symptoms and discomfort have already appeared, and in this case it is already too late. If astheno-vegetative syndromes appear, that is, weakness, malaise, sweating, and so on, then the person should immediately bring this to the attention of a health care worker.

Today, women over 25 years of age are required to undergo mammography, and the same applies to the prevention of cervical cancer. That is, this is a mandatory check with a gynecologist. If we take men after 30 years of age, then this is the prevention of prostate diseases. Mandatory visit to an andrologist and urologist. If we talk about intestinal diseases, then there is rectal cancer, which means that you need to visit a proctologist for preventive purposes. Lung cancer must be detected by undergoing mandatory fluorography every year. This is especially true for those who work in enterprises with specific production, such as hot shops. Here you need to be examined 100%. As for stomach cancer, those over 35 years old should have their stomach examined for prevention. In any case, there are harbingers.

- To what extent is the republic provided with medicines in the area of ​​oncology today?

There are no problems with drugs at all. There are problematic issues related to high-tech drugs. Unfortunately, it must be said frankly that these medicines, as a rule, are imported; there are no Russian analogues. The cost of such drugs is, of course, high, but in this situation it literally means life expectancy. I am grateful today to the state program and the republican budget that are addressing this issue.

- When will the oncology dispensary be completed?

We are now waiting for the order of the Russian government. We received information that the Ministry of Finance has approved a construction project with funding for the next three years. So, in 2018, the estimated amount that we will be given for construction from the federal budget is 734 million, plus the republic will allocate some part. This is enough for us to complete the construction of the oncology clinic itself. Then in 2019-20, a billion funds will be allocated for the construction of the stationary part. We need to look at how much financial resources will be spent on construction, installation work, how much on medical equipment, and so on. Because there is simply no other money, and there is unlikely to be any more, so the allocated funds need to be distributed as rationally as possible.

- Valery Veniaminovich, how do you treat yourself? Do you see a doctor or do it on your own?

At the Ministry of Health, we also undergo strict medical examinations. In particular, we are now undergoing it in the first clinic. We also vaccinate medical workers, and I, among other things, go there and get vaccinated. I myself am assigned to clinic No. 6 at my place of residence, and I go there if necessary.

Well, let’s say you have a sore throat, dizziness and other symptoms, what do you do in this case?

I think that I can cope with these symptoms, given my work experience. As for the examination, I believe that yes, indeed, we must definitely donate blood for a general analysis and biochemistry. Fluoragraphy and ultrasound should be performed once a year.

- How many children do you have and have they followed in your footsteps?

Not all. My eldest son is a manager, he works in Vladivostok, his name is Evgeniy. My daughter Ekaterina followed in my footsteps and now works in Novosibirsk as an x-ray diagnostics doctor, although she is now on maternity leave. My youngest daughter is now finishing 11th grade, but she won’t follow in my footsteps either. I do not want to impose my point of view, and have never done so, on the issue of choosing a profession. I think that you should love your profession and be fascinated by it. The profession of a doctor is considered beautiful, but at the same time you need to understand what responsibility falls on the doctor’s shoulders. This is a specialty that requires full dedication and constant work.

The highest requirements

Such an abundance of strong candidates for the competition for the Minister of Health distinguishes it favorably from other competitions. But the requirements for the position of Minister of Health at the moment are perhaps the highest. The Ministry of Tourism has just been created, and the new minister will most likely be given time to get going. The spheres of culture and sports have been rocked by scandals in recent years in the absence of any super results. And in any case, the “low base” effect will work. The functions of the Ministry of Economy are partially blurred, and the main responsibility for the economic sector still lies with the supervising deputy chairman.

But the new head of the Ministry of Health will have to work in the “here and now” mode. The issue with personnel is acute: almost half of the doctors are over 50 years old, and there is a catastrophic shortage of new young personnel. This is largely due to low wages. Complaints are heard everywhere about huge queues to see “narrow” specialists. The technical base is very outdated. To solve these problems, the new minister will have to be a politician, diplomat and business executive at the same time. All that remains is to wish him good health!
